Why the sample is processed here and not sent away

Sperm motility begins to fall within minutes of collection, and it keeps falling with every drop in temperature and every hour of transport. A semen sample is the least forgiving specimen in a routine laboratory. Most diagnostic outlets around Gaur Chowk and 1 Murti are collection points — the sample is packed and driven to a central hub, often in Noida. At Index Path Labs the sample is analysed on the same floor it is received on, and the report is read by a pathologist who is physically in the building.

How to prepare

  • Abstain from ejaculation for 2 to 7 days. Less than 2 days lowers volume and count; more than 7 days lowers motility. Both distort the result.
  • No lubricants and no ordinary condoms. Both are toxic to sperm and will invalidate the test.
  • Mention recent fever, illness or antibiotics when you book. A fever in the previous two to three months can measurably change the result, and it is worth recording on the request.
  • Avoid alcohol for 2 to 3 days before the test where you can.

How collection works

The sample is produced by masturbation into a sterile container we provide. A separate collection room is available at the lab. Collection at the laboratory is preferred, because motility has to be assessed inside a limited window and the sample needs to stay at body temperature. If you need to collect at home, the sample must reach us within about 30 to 45 minutes and be kept close to the body on the way — please discuss this when you book rather than assuming it will be accepted.

Samples are labelled by number, not by name, and the report is released to you directly.

What the report measures, and the WHO 2021 reference values

These are the lower reference limits published in the sixth edition of the WHO laboratory manual (2021). It is worth knowing exactly what they are: each figure is the 5th centile of roughly 3,500 men whose partners conceived naturally within a year of trying. In other words, about one man in twenty who fathered a child without any medical help fell below each of these numbers.

They are not a pass mark. A value below the limit does not on its own mean infertility, and a value above it does not rule out a problem. The sixth edition made this point emphatically by removing the diagnostic labels “normozoospermia” and “asthenozoospermia” from the manual entirely, on the grounds that no single threshold means anything in isolation. This is precisely why the report is read by a pathologist alongside your history rather than simply checked against a column of numbers.

If the first report is abnormal, the standard practice is to repeat it after a gap of a few weeks before drawing any conclusion. My report is abnormal. What does that mean?

Usually that it needs repeating before anything is concluded from it. Semen parameters fluctuate substantially in the same man from one week to the next, and a single low reading after a fever, an illness or a short abstinence period is common and often not meaningful. Standard practice is a repeat test after a few weeks. Take both reports to your treating doctor — interpretation and any treatment belong with them, not with the laboratory.

Which reference ranges do you report against?

The sixth edition of the WHO laboratory manual, published in 2021. If you are comparing an older report from another laboratory, check which edition it used — the 2010 fifth edition limits are slightly different, and the two are not directly comparable.
