What the panel measures
- Serum creatinine — a waste product from muscle, cleared by the kidneys. The core value, though it is affected by muscle mass, which is why the same figure means different things in a bodybuilder and a frail elderly patient
- Blood urea and BUN — also cleared by the kidneys, but influenced by protein intake, dehydration and bleeding, so less specific on its own
- eGFR — an estimate of filtration rate calculated from creatinine with age and sex. Usually the most clinically useful number on the report
- Uric acid — relevant to gout as well as kidney function
- Electrolytes — sodium, potassium, chloride — the balance the kidneys maintain. Potassium in particular matters, and it is one of the values most distorted by a poorly handled sample
- Calcium and phosphorus, often included, as both are affected in longer-standing kidney disease
Why silence is the problem
The kidneys have substantial reserve. Function can fall considerably before anyone feels different, and by the time symptoms appear — swelling, reduced urine output, persistent fatigue, nausea — the damage is often well established and not fully reversible.
The groups where periodic testing is usually advised:
- Diabetes — the leading cause of kidney disease in India
- High blood pressure — the second
- Family history of kidney disease
- Long-term use of painkillers, particularly NSAIDs
- Anyone over 40 having a general health checkup
If you have diabetes or hypertension, kidney testing is not an optional extra. Ask your doctor what interval they want.
Urine matters as much as blood
A urine routine and microscopy, and specifically urine protein, often detects kidney involvement earlier than creatinine does. Protein appearing in urine is frequently the first sign in diabetic kidney disease, at a point where creatinine still reads normal.

Preparation
An 8 to 12 hour fasting sample is generally preferred, particularly when KFT is bundled with sugar and lipids. Be normally hydrated — dehydration raises urea and creatinine and can make kidney function look worse than it is. Avoid a heavy protein meal the night before, and mention any painkillers or supplements you take regularly.

How soon will the report arrive?
Usually within 6 to 8 hours; before 9 AM collection is typically reported the same evening.
My creatinine is slightly high. Is that kidney disease?
Not on its own. Dehydration, a high-protein diet, heavy exercise and high muscle mass all raise it. Your doctor will look at eGFR, urine protein and a repeat sample before concluding anything.
