A low haemoglobin tells you anaemia is present. It does not tell you why — and treating the wrong cause is worse than not treating at all. Iron studies are the tests that answer the second question. Why haemoglobin alone is not enough
Iron deficiency is the most common cause of anaemia in India, but it is far from the only one. Vitamin B12 and folate deficiency, thalassaemia trait, chronic disease, kidney disease, thyroid disorders and blood loss all produce a low haemoglobin, and they need entirely different treatment.
Iron tablets help iron deficiency. They do nothing for B12 deficiency, nothing for thalassaemia trait, and taken unnecessarily over months they can cause harm. This is why the follow-up test matters.
Start with the MCV on your CBC
Before ordering anything else, look at the MCV — mean corpuscular volume — on your existing CBC report. It is the cheapest clue you already own:
- Low MCV — small cells. Points towards iron deficiency or thalassaemia trait
- High MCV — large cells. Points towards vitamin B12 or folate deficiency
- Normal MCV — points towards chronic disease, recent blood loss, or two causes cancelling out

What the iron panel measures
- Serum ferritin — the body’s iron stores, and the single most useful test for iron deficiency. Important caveat: ferritin also rises with any inflammation or infection, so a normal ferritin during an illness does not exclude deficiency
- Serum iron — circulating iron, which varies considerably through the day and with recent meals
- TIBC and transferrin saturation — how much capacity there is to carry iron, and how much of it is filled. Low saturation with high TIBC is the classic iron-deficiency pattern
- Vitamin B12 and folate — usually added when MCV is high
Preparation
A morning fasting sample is preferred for serum iron, because levels are highest in the morning and fall through the day, and iron-rich food or supplements shortly before the test can distort the result considerably.
If you are already taking iron tablets, tell your doctor before testing. Supplements raise serum iron for hours after a dose, and some doctors ask patients to pause for a few days before a repeat test — but do not stop prescribed medication on your own.
Who should consider testing
- Persistent tiredness, breathlessness on stairs, or unusual pallor
- Heavy menstrual bleeding — the most common cause of iron deficiency in women here
- Pregnancy, where anaemia is both common and consequential
- Vegetarian and vegan diets, particularly alongside low B12
- Known low haemoglobin on a previous report that was never investigated further
- Hair fall and brittle nails, which are worth checking alongside thyroid and vitamin D
Frequently asked questions
How soon will results arrive?
Ferritin and the iron panel are usually available the next morning. CBC is same day.
My haemoglobin is normal but I am exhausted. Is testing pointless?
No. Iron stores fall before haemoglobin does, so ferritin can be low while haemoglobin is still in range. That stage is worth catching.
