Semen Analysis — Preparation, What the Report Measures, and Why One Test Is Not Enough
A semen analysis is the first test in almost any fertility assessment, and one of the most straightforward. Index Path Labs offers discreet collection at the lab in Arza Square 2, Gaur City 1, with reports reviewed by Dr. Ayesha (DNB Pathology) and released confidentially.
Roughly a third to a half of fertility difficulty involves a male factor, so testing both partners early avoids months of investigating one side alone.
Preparation, which genuinely affects the result
- Abstinence of 2 to 7 days before the sample. This window matters. Less than two days tends to lower the count; more than seven tends to reduce motility. Three to five days is a reasonable target.
- Avoid alcohol for a few days beforehand where you can
- Mention any recent fever or illness. A fever can suppress sperm production for weeks afterwards, and a poor result taken shortly after flu may say more about the flu than about fertility
- List medicines and supplements, including anything for hair loss or testosterone, which can affect results significantly
- Mention recent antibiotics or any treatment in the past three months
How collection works
The sample is produced by masturbation into a sterile container provided by the laboratory. Lubricants and ordinary condoms should not be used, as both can be toxic to sperm and invalidate the result.
Collection at the lab is preferred because motility must be assessed within a limited window and the sample needs to be kept at body temperature. If collection at home is necessary, it has to reach the lab within about 30 to 45 minutes, kept close to the body — discuss this when you book rather than assuming.
The sample is handled discreetly, labelled by number, and the report is released to you directly.
What the report measures
- Volume — how much semen was produced
- Concentration — sperm per millilitre
- Total sperm number — concentration multiplied by volume
- Total and progressive motility — what proportion are moving, and what proportion are moving purposefully forward. Progressive motility is the more meaningful figure
- Morphology — the proportion with normal shape, assessed against strict criteria. The normal threshold is far lower than people expect, and a low percentage alone is rarely the whole story
- Vitality — the proportion alive, assessed when motility is very low
- pH, liquefaction time and white cells — the last can suggest infection
Reference values follow World Health Organization criteria. Those thresholds describe the lower boundary of a reference population — they are not a line between fertile and infertile, and men below them do conceive.
One result is not a conclusion
Semen parameters vary substantially in the same man from week to week, influenced by illness, sleep, stress, heat exposure and abstinence period. A single below-threshold result is a reason to repeat the test, not to draw conclusions from.
Standard practice is to repeat after an interval — often around 6 to 12 weeks, since sperm production takes roughly 70 to 90 days, so a sample today reflects conditions two to three months ago. If a recent fever or illness might explain a poor result, that timing matters.
Take both reports to a doctor together. A urologist or fertility specialist reads them alongside examination and history.

Is the test uncomfortable?
No. It involves no needles, no examination and no procedure. The most common difficulty patients report is awkwardness rather than discomfort, and the process is handled without fuss.
Who sees the report?
It is released to you. Reports are not discussed with family members or anyone else without your consent.
Can I collect the sample at home?
Sometimes, if you can reach the lab within about 30 to 45 minutes with the sample kept warm. Lab collection gives a more reliable motility assessment. Ask when you book.
How long until I get the report?
Usually the same day.
