Lipid Profile Test Explained — LDL, HDL, Triglycerides & Ratio

What each value on the report means

  • Total cholesterol — the headline number, and the least useful on its own. It combines everything below, including the type you want more of.
  • LDL cholesterol — often called “bad” cholesterol. LDL carries cholesterol into artery walls, where it contributes to plaque. This is the number most treatment decisions are based on.
  • HDL cholesterol — “good” cholesterol. HDL carries cholesterol away from the arteries back to the liver. Here, higher is better, which is why a raised total cholesterol driven by high HDL is a different situation from one driven by high LDL.
  • Triglycerides — a separate type of fat, strongly affected by recent food, alcohol, refined carbohydrate and uncontrolled diabetes. Often the value that moves most with lifestyle change.
  • VLDL — usually calculated rather than measured directly, derived from triglycerides.
  • Total cholesterol to HDL ratio — often more informative about cardiovascular risk than any single number.

Why the ratio matters more than the total

Two people can have identical total cholesterol with quite different risk. One might have high LDL and low HDL; the other high HDL and normal LDL. The total looks the same; the picture is not.

This is why a doctor reads the whole panel rather than the top line, and why a report that shows only total cholesterol is of limited use.

Fasting: how long, and why

A 9 to 12 hour fast is standard for a lipid profile. Triglycerides in particular rise after eating and can stay elevated for hours, so a non-fasting sample can read misleadingly high.

Water is allowed and encouraged throughout — being well hydrated makes the blood draw easier. Black tea or coffee without sugar or milk is generally acceptable, but check when you book. Alcohol should be avoided for 24 hours beforehand, as it raises triglycerides sharply.

What moves these numbers

Genetics set a large part of your baseline, and no amount of lifestyle change fully overrides familial hypercholesterolaemia. But several things do shift the values:

  • Triglycerides respond quickly to reduced refined carbohydrate, sugar and alcohol, and to weight loss
  • HDL rises modestly with regular aerobic exercise and falls with smoking
  • LDL responds to reduced saturated and trans fat, increased soluble fibre, and to medication where indicated
  • Uncontrolled diabetes and untreated hypothyroidism both worsen the lipid picture, which is why they are often tested alongside

If your result is abnormal, the useful next step is a conversation with your doctor about your overall cardiovascular risk — which factors in age, blood pressure, diabetes, smoking and family history, not cholesterol alone.

  • Adults from around age 25 to 30, as a baseline, then as advised
  • Anyone with diabetes, high blood pressure, obesity or a family history of early heart disease
  • People already on lipid-lowering medication, at the interval their doctor sets

Can I take my medicines during the fast?

Usually yes, with water. Do not skip prescribed medication to fast — ask your doctor or tell us when you book, and we will note it.

How soon will the report arrive?

Lipid profiles are typically ready within 6 to 8 hours of collection. Samples taken before 9 AM are usually reported the same evening on WhatsApp.

My cholesterol is borderline. Do I need medication?

That is not a decision a number makes on its own. Treatment depends on your total cardiovascular risk, not the lipid panel in isolation. Take the report to your doctor.
