CBC Blood Report Explained — Haemoglobin, MCV, WBC & Platelets

A Complete Blood Count is the most commonly ordered blood test there is, and the most commonly misread. It counts the three cell types in your blood — red cells, white cells and platelets — and reports a dozen or more values alongside them. This page explains what those values describe, so the report makes sense before you see your doctor.

Haemoglobin — the oxygen carrier

Haemoglobin is the protein inside red cells that carries oxygen from your lungs to the rest of your body. It is the value most people look for first, and the one most often low in this region.

Low haemoglobin is called anaemia. It has many causes — iron deficiency is the most common in India, but B12 or folate deficiency, chronic blood loss, thyroid disease and kidney disease all cause it too. The CBC tells you anaemia is present; it does not tell you why. That usually needs iron studies, vitamin B12 or a ferritin test alongside.

MCV — the clue most people miss

MCV, or mean corpuscular volume, is the average size of your red cells. It is the single most useful number for narrowing down the cause of anaemia, and it is routinely ignored.

  • Low MCV — small red cells. Points towards iron deficiency or thalassaemia trait, which is not rare in this population.
  • High MCV — large red cells. Points towards vitamin B12 or folate deficiency, and sometimes thyroid or liver problems.
  • Normal MCV with low haemoglobin — points towards chronic disease, recent blood loss, or a mixed deficiency where two causes cancel each other out.

If your haemoglobin is low, look at the MCV next. It usually tells your doctor which test to order second.

White cell count — the infection signal

White cells fight infection. The total count matters less than the differential, which breaks it into types:

  • Neutrophils rise in bacterial infection
  • Lymphocytes often rise in viral infection
  • Eosinophils rise in allergy and parasitic infection — common enough here to be worth noting
  • Monocytes rise in some chronic infections

A raised total white count with a neutrophil predominance suggests a different problem from a raised count with lymphocyte predominance. That is why the differential is reported, and why a doctor reads it rather than just the headline number.

Platelets — and why the trend matters more than the value

Platelets help blood clot. A single low reading during a fever illness is far less informative than the direction across three days, which is why dengue patients are asked to repeat the test. See our guide on dengue testing and platelet monitoring at home.

Platelet counts can also read falsely low if the sample clots slightly in the tube — one of several reasons sample handling changes results. A surprising platelet result is worth confirming before anyone acts on it.

ESR and the peripheral smear

ESR is a non-specific marker of inflammation. It rises in infection, inflammation, pregnancy and anaemia, so a raised ESR tells you something is going on without telling you what. It is useful alongside other results, not on its own.

The peripheral smear is a pathologist looking at your blood cells under a microscope. It picks up abnormal cell shapes, parasites such as malaria, and patterns an automated analyser can miss. At Index Path Labs the smear is reviewed by Dr. Ayesha (DNB Pathology, Oncopathologist).

Reference ranges are not verdicts

Every value on your report sits next to a reference range, and it is tempting to read anything outside it as a problem. Reference ranges are built so that roughly 95% of healthy people fall inside them — which means about one in twenty healthy people falls outside on any given value, by definition.

A haemoglobin marginally below range in someone who feels well means something different from the same value in someone breathless on stairs. Ranges also differ by age, sex and pregnancy. This is why a result needs a doctor, not a search engine.

Frequently asked questions

Do I need to fast for a CBC?

No. A CBC can be taken at any time of day with no fasting. If a fasting sugar or lipid profile is being done at the same visit, those do need an 8 to 12 hour fast.

How quickly will I get the report?

CBC is usually reported within 6 to 8 hours of collection. Samples collected before 9 AM are typically reported the same evening, on WhatsApp.

My haemoglobin is slightly low. Do I need iron tablets?

Not necessarily, and not without knowing the cause. Iron supplements help iron-deficiency anaemia and do nothing for B12 deficiency or thalassaemia trait — and iron taken unnecessarily can cause harm. Take the report to your doctor.
