Urine routine is one of the most ordered tests in medicine and the one most often ruined before it reaches the laboratory. How the sample is collected changes the result more than almost any other routine test. How to collect a clean sample
The standard is a midstream clean-catch, first sample of the morning where possible, because the first morning urine is most concentrated and abnormalities show up more reliably.
- Wash your hands, then clean the genital area with plain water and dry it
- Begin passing urine into the toilet
- Without stopping, move the container into the stream and collect the middle portion
- Finish passing the rest into the toilet
- Close the container without touching the inside of the lid or rim
- Get it to the laboratory within about an hour, or refrigerate it — cells and casts break down at room temperature, and bacteria multiply, which can turn a normal sample into a falsely abnormal one
The midstream part matters because the first portion washes out organisms from the urethra that are not from the bladder. Skipping it is the single most common reason a urine culture comes back with mixed growth and has to be repeated.
Women should avoid collecting during menstruation if possible, as blood contaminates the result. If it cannot wait, tell us so it can be noted.
What the report covers
- Physical — colour, appearance, specific gravity, which reflects concentration and hydration
- Chemical — protein, glucose, ketones, blood, bilirubin, urobilinogen, nitrite, leukocyte esterase and pH
- Microscopy — pus cells, red cells, epithelial cells, crystals, casts, bacteria and yeast, examined under a microscope
The values that usually matter most
- Protein — often the earliest sign of kidney involvement in diabetes and hypertension, appearing before creatinine rises. See our kidney function guide
- Pus cells with nitrite or leukocyte esterase — suggests urinary infection, usually followed by a culture to identify the organism
- Glucose — can be the first pointer to undiagnosed diabetes
- Blood — needs explanation, from infection and stones through to causes that require prompt review
- Ketones — appear in prolonged fasting and in poorly controlled diabetes
Culture and sensitivity
If infection is suspected, a urine culture identifies the organism and which antibiotics it responds to. The sample should ideally be collected before antibiotics are started — once treatment begins, the culture often grows nothing even when infection is present. Worth raising with your doctor before a prescription is written.
Culture takes around 48 hours by nature; routine and microscopy are usually same day.
Frequently asked questions
Do I need to fast?
No. If a fasting blood test is being done at the same visit, collect the urine at home that morning and hand it over with the blood sample.
Can I use my own container?
Please don’t. A washed household container leaves detergent and bacteria that affect results. We provide a sterile container.
How soon will the report arrive?
