What the panel measures
- SGPT (ALT) and SGOT (AST) — enzymes released when liver cells are injured. ALT is the more liver-specific of the two; AST also comes from muscle, so vigorous exercise before a test can raise it
- ALP (alkaline phosphatase) — rises when bile flow is obstructed, and also comes from bone, which is why it is naturally higher in growing children
- GGT — often used alongside ALP to work out whether a raised ALP is coming from liver or bone. Also sensitive to alcohol
- Bilirubin, total and direct — the pigment that causes jaundice. The split between direct and indirect helps localise the problem
- Total protein, albumin and globulin, with the A/G ratio — albumin is made by the liver, so a persistently low level suggests longer-standing impairment rather than acute injury
Why it is ordered
- Fatty liver, which is increasingly common and often first suspected on an ultrasound done for another reason
- Routine monitoring for people on long-term medication — several drug classes require periodic liver checks
- Jaundice, unexplained fatigue, right-sided abdominal discomfort, itching, or dark urine
- As part of a general health checkup, which is where most abnormal results are first picked up
- Alcohol-related assessment
Interpreting a mildly abnormal result
Mildly raised liver enzymes are common and frequently not serious. Fatty liver, a recent viral illness, a new medication, a heavy gym session before the test, or alcohol in the preceding days can all do it.
The pattern matters more than any single value. A raised ALT with normal bilirubin reads differently from raised ALP and bilirubin together. And a repeat test after a few weeks tells you more than a single reading — many mild elevations settle on their own.
What is not sensible is either ignoring an abnormal result or panicking about it. Take it to your doctor, who will look at the pattern, your history and your medications together.
Preparation
Fasting is not strictly required for LFT alone, but an 8 to 12 hour fasting sample is preferred, partly because a recent fatty meal can affect some values and partly because LFT is usually taken alongside a lipid profile or fasting sugar that does require it.
Avoid strenuous exercise for 24 hours beforehand, as it can raise AST. Tell us about alcohol in the previous few days, and list your regular medicines and supplements — several affect these values.
