Why HbA1c rather than a fasting sugar

A fasting glucose is a snapshot of one morning. It moves with what you ate last night, how you slept, whether you were unwell. HbA1c measures the proportion of your haemoglobin that has glucose attached to it — and because red cells live around three months, it reflects the average across that whole period.

That makes it far harder to game. A patient who eats carefully for two days before a test can move a fasting sugar. They cannot move an HbA1c.

What the number represents

HbA1c is reported as a percentage. Broadly, the higher the figure, the higher your average glucose has been. Your doctor will interpret it against the current diagnostic thresholds alongside your symptoms, weight, family history and any fasting or post-prandial results.

Two things are worth knowing:

  • It can be misleading in some conditions. Anaemia, recent blood loss, blood transfusion, chronic kidney disease, pregnancy and some haemoglobin variants all affect the result, because they change red cell lifespan. If you have any of these, tell your doctor — a fasting glucose or an OGTT may be more reliable for you.
  • It is an average, not a range. Someone with wide daily swings can show the same HbA1c as someone whose sugar is steady. This is why doctors often want both HbA1c and a fasting or post-prandial value.

How often to test

  • Diagnosed diabetes, stable — commonly every 6 months, as your doctor advises
  • Diabetes with recent medication change or poor control — commonly every 3 months
  • Prediabetes — usually annually
  • Screening — worth discussing from around age 35, or earlier with obesity, family history, hypertension or PCOS

Testing more often than every three months rarely adds information, because the marker itself takes that long to move.

Preparation
