What this test measures
Prostate Specific Antigen is a protein made by the prostate gland. Levels rise with prostate enlargement, inflammation and prostate cancer alike, so PSA is a prompt for further assessment rather than an answer in itself – your physician reads it alongside age, examination and symptoms.
Who should consider it
Men over 50 as part of routine screening, and from 45 where there is a family history of prostate cancer. Also relevant for men with weak urinary stream, difficulty starting, dribbling, or needing to get up repeatedly at night.
Preparation
No fasting required, but avoid ejaculation, cycling and vigorous exercise for 48 hours before the sample, and schedule the test before – not after – any digital rectal examination or catheterisation, as all of these can raise PSA artificially.
